For the convenience of our customers, we have provided all of the forms that make up our Employee New Hire Kit below.  If you are looking for additional forms, click the "More Forms" link to the left of the page.

Each of the forms listed on this site are in the Adobe format, meaning that you must have Adobe Acrobat© Reader installed on your computer to view and print them.  This software can be downloaded FREE from the Adobe web site by clicking here: 

To open a form, just click on its title and Adobe Acrobat Reader will open and display the form.  Select Print from within the Adobe Acrobat Reader program to print the form.  Use your Browser's BACK button to return to this page.

New Hire Kit

These are the forms that must be completed by each new employee you hire.  The New Hire Employee Kit (cover letter) provides both Supervisor and Employee directions for completion of each form.  When returning the kit, check to make sure that each of the 5 forms have been fully completed and signed.
