Allegiant has numerous forms available for use by our clients to help lighten the burden of their everyday record keeping chores.  All of these forms have been converted into the Adobe PDF format.  If you have a need for any of the forms listed below, simply call our Human Resources Department and we will email you a PDF version of the form that you can download and then print whenever you need it.

To make locating a form easier, we have grouped them by category and listed them alphabetically by title 

Payroll Forms

These are forms that some of our clients use to report employee payroll hours and to initiate other necessary payroll actions such as authorizing payroll deductions, etc.  If you have an existing form you would like to use, let us know and we will try and work with you in its use.
